How to Submit Supplementary & Subject Matter Authorization Request Form
To submit the online Supplementary & Subject Matter Authorization request form, follow the steps below:
Step 1: Complete request form
Complete the online Request for Supplementary & Subject Matter Authorization form and attach your transcripts as applicable. Attachment is not required if you have all transcripts on file with SDSU.
Step 2: Electronically sign form
Electronically sign your request and click on “Sign Form” to send in your request.
Step 3: Validate email address
To successfully send your request electronically to the Office of Student Services, you must validate your email address as the final step to confirm submission of your request.
