Instructions for Those Without an Active mySDSU Account

Welcome to the alternative submission process for the Credential Exit Evaluation form. These instructions are only for Inactive Credential Students who have lost access to the my.SDSU system and their SDSU email. Current Credential Students should NOT use this process. The instructions below will guide you through completing and submitting your form via EchoSign (Adobe Sign) to ensure your evaluation is processed without delay.

Why can't I use the standard my.SDSU/ONBASE form?
The university deactivates your SDSU email account 6 months after your program completion date. Since your SDSU email is linked to the my.SDSU system, you are considered an Inactive Credential Student and can no longer access the standard ONBASE form to submit your evaluation request.

How do I submit my Credential Exit Evaluation Request?

You must submit your Credential Request Form electronically to the Office for Student Success using the EchoSign application process.

Payment and Submission Requirements

Submit the Credential Application/Evaluation Fee

  • The required Credential Evaluation Fee is $25.
  • Pay directly at SDSU E-Cashier.
  • You must save the payment receipt, as you will need to upload it with your request form.


The 3-Step EchoSign Submission Process

Follow these steps carefully to ensure your request is submitted successfully:

Step 1: Complete and Attach Documents

  • Fill out the Credential Request Form completely (see Choose the Correct Form for Your Program).
  • UPLOAD: You must upload the receipt of your $25 credential fee payment (obtained from CASHNET).
  • ATTACH: Include any other required items listed on the checklist that you have not previously submitted to the office.

Step 2: Electronically Sign and Send

  • Once the form is complete, electronically sign your request.
  • Click on "Sign Form" to initiate the submission.

Step 3: Validate Email Address (Crucial Final Step)

  • You MUST validate your email address to send your request to the Office for Student Success successfully.
  • Immediately check the email address you used on the form after clicking "Sign Form."
  • Look for a validation email from: [email protected] 
  • IMPORTANT: This email may be filtered into your Junk or Spam folder. Please check these folders thoroughly.
  • Follow the instructions in that email to validate and confirm your submission.
  • Once your email address is validated, your EchoSign (Adobe Sign) form will be forwarded to our office for processing. Please note that you will not receive a separate confirmation email indicating that your Credential Exit Evaluation form was received.
  • If you would like confirmation, you may contact your assigned analyst directly.
