Request for Subject Matter Preparation Program (CSET Waiver Letter)

Although completion of a Waiver Program is no longer required to meet Subject Matter preparation requirements in the state of California, some individuals may require proof of having completed a waiver program for enrollment at institutions outside of California. 

How to Submit the Subject Matter Preparation Program (CSET Waiver Letter) Request Form

Step 1: Complete Subject Matter Preparation (CSET waiver) form

Complete the online Subject Matter Preparation Program (CSET waiver letter) request and attach the multiple or single subject credential program advisor’s recommendation form signed by your subject matter waiver program advisor.

Step 2: Electronically sign form

Electronically sign your request and click on “Sign Form” to send in your request.

Step 3: Validate email address

To successfully send your request electronically to the Office for Student Success, you must validate your email address as the final step to confirm submission of your request.
