Instructions for Current Credential Students (Fall and Spring Finishers)

How to Submit the Internship Credential Evaluation?

Internship candidates submit their Internship Credential Evaluation through their mySDSU account using the OnBASE form. This process is separate from the Credential Exit Evaluation and is required for students seeking an Intern Initial Credential Document.

Submission Process

The Internship Credential Evaluation requires two steps:

Step 1: Pay the Internship Credential Evaluation Fee

Step 2: Submit the Internship Credential Evaluation Form

Follow the steps below to submit your internship request:

  1. Log in to my.SDSU and select the Academic Records tile.
  2. In the left navigation panel, select Student Records Forms.
  3. Select the Credential Students-Credential Evaluation Form.
  4. Read and follow all the instructions listed on pages 1, 2 and 3 of the Credential Request Form.
  5. On the form:
    • Under “Anticipated Program Completion Term,” please specify your projected completion term (for example: Spring 2026, Summer 2026). This should reflect your best projection of when you expect to finish all program requirements based on your program enrollment (part-time or full-time).
    • Under “I am applying for…”, select Intern Initial Document.
    • Under Type of Credential, select the program in which you are currently matriculated.
      • Education Specialist (MMSN, ESN, ECSE)
      • Multiple Subject
      • Single Subject
  6. Complete all required fields on page 2 of the form and upload:
    • Your $25 fee payment receipt
    • Any additional required attachments as applicable. Please note, you may submit your internship credential request form without an internship job offer. Subsequent materials can be sent to your assigned credential analyst at a later date.
  7. Click Submit Form. A confirmation email will be sent immediately.
  8. You will receive an SDSU email notification once your assigned credential analyst has reviewed your internship application.
  9. Continue checking your SDSU email for updates from the Office for Student Success / Credentialing Office.
